In the world of forensic medicine, not everything that looks like violence is violence, and not everything that resembles injury actually is one. Courts, investigators, and even seasoned lawyers can be misled by the body's own postmortem behavior — changes that occur after death but mimic the signs of trauma or struggle. Two striking examples illustrate this phenomenon: the heat haematoma found in burn victims and the sanguineous purging fluid seen in hanging cases. Understanding these is not merely an academic exercise for pathologists; it is essential knowledge for judges, prosecutors, and defence counsel who must correctly weigh medical evidence rather than be swayed by its outward appearance.

The Heat Haematoma: Fire's Deceptive Signature

When a body is subjected to intense heat, such as in a fire, the skull's protective lining — the dura mater — reacts to the thermal assault. The heat causes the dura to contract and peel away from the inner surface of the skull. As it separates, blood is drawn out from the venous sinuses embedded within the bone, and this exuded blood clots within the extradural (epidural) space.

The result is a mass that looks strikingly similar to a genuine extradural (epidural) haemorrhage — the kind caused by traumatic rupture of the middle meningeal artery, a classic sign of head injury sustained before death. But the resemblance is only skin-deep. A heat haematoma is:

·      Firm and honeycomb-like in texture, rather than a smooth, fresh clot

·      Often brick-red to chocolate-brown in colour, unlike the uniform red of a true traumatic bleed

·      Mixed with melted fat globules from bone marrow, a telltale sign of heat exposure

·      Completely unrelated to the middle meningeal artery or any traumatic vascular rupture

·      Devoid of any "vital reaction" — the biological response only a living body can produce

This last point is the crux of the forensic distinction. A true epidural haemorrhage occurs while the person is alive, triggering inflammatory and healing responses in the tissue. A heat haematoma forms after death, purely as a passive consequence of thermal injury to a body that can no longer react.

Blood From the Mouth and Nose: Decomposition's False Alarm

A similarly deceptive artifact appears in cases of hanging. Investigators and family members alike are often alarmed to see blood-tinged, frothy fluid trickling from the deceased's nose or mouth, and it is not uncommon for this to be misread as a sign of assault, a struggle, or foul play.

In reality, this fluid is frequently nothing more than decomposition purging. As putrefaction sets in, gases generated by bacterial activity build up inside the body's cavities. This internal pressure pushes fluids — often blood-tinged due to breakdown of blood vessels and tissue — up through the natural passages of the nose and mouth. It is a normal, expected postmortem change, not evidence of antemortem violence.

The Common Thread: Understanding "Artifacts"

Both phenomena belong to a broader forensic category known as artifactspostmortem changes that mimic real injury or pathology but arise from external factors such as heat, decomposition, embalming, or even handling of the body during recovery or autopsy, rather than from anything that happened while the person was alive.

An artifact is, in essence, a false signal. It carries the visual language of trauma without any of its substance. Forensic artifacts are typically grouped into three broad categories:

·      Heat-induced artifacts — such as heat haematomas or the pugilistic posture (boxer-like stance) seen in severely burned bodies

·      Decomposition artifacts — such as purging fluid, skin slippage, or bloating that can superficially resemble bruising

·      Mechanical or procedural artifacts — marks left by resuscitation attempts, body transport, or the autopsy process itself

Why This Matters in the Courtroom

The distinction between a genuine injury and a postmortem artifact is not a mere technicality; it can determine the entire trajectory of a criminal investigation or trial. Misreading a heat haematoma as evidence of a pre-fire head injury could wrongly suggest that a victim was assaulted before a fire was set to conceal the crime. Misreading decomposition purging as blood from a struggle could wrongly cast suspicion on a suicide as a homicide.

The defining markers that separate a true injury from an artifact are consistent: the absence of a vital reaction, and the presence of a plausible, non-violent explanation rooted in environmental conditions — fire, putrefaction, or handling — rather than in any act inflicted upon a living person.

Conclusion

Forensic science's contribution to the justice system lies as much in what it rules out as in what it proves. The heat haematoma and decomposition purging fluid are reminders that the body, even after death, can produce convincing illusions of violence. For judicial officers, prosecutors, and advocates, the ability to recognise these artifacts — and to demand that expert witnesses clearly distinguish them from genuine trauma — is indispensable to arriving at sound, evidence-based conclusions rather than being misled by appearances.
